2022 Update: Interactive Medical Data Breach Map

The map below displays the location of each medical data breach recorded at the US Department of Health and Human Services from 2009-2022.

To get the most from this map, you can view it multiple ways; by year, by region, and in a text list.

  • View medical breaches by year: Select the Show Year drop down menu. You can choose Show All to see all years of breaches at once, or select years singly, for example, select 2009 or 2022.
  • View medical breaches by region: Select the List by Region drop down menu. You can choose a specific state in the US, such as New York, California, or Illinois from the drop down menu.
  • View a text list of medical data breaches: Click the List All button to see a text list of medical breaches from 2009-February 2022.

If you select a year and a state, you will further refine the results.

To clear your past searches, click the Reset button.

If you are on a device using a trackpad or a mouse to navigate, hover over each breach to see a pop-up of information about that breach. For mobile devices, the breach information for your selection will be listed just below the map in a text list.

The data for this map is sourced from the US Department of Health and Human Services, pursuant to its obligation under section 13402(e)(4) of the HITECH Act to post a list of breaches of unsecured protected health information affecting 500 or more individuals. The HHS medical breach list is here.
